SHP has supported City and Hackney tPCT to plan the redevelopment of St Leonard's Hospital site to accommodate a progressive range of Primary and Community based services as a Resource Centre for the local population.

This included an extended Primary Care service, Physical Disability services, Sexual Health and Women's Health services, Diagnostic services, a referral management centre and enhanced services for older people.

The drivers for services change were to improve access to services, change the range and style of services to meet the specific needs of the local population, improve the quality of service delivery and support the predicted increase in people living within the tPCT area.

SHP initially carried out two strands of work: a Best Practice review and a six facet survey of the current estate. This supported the tPCT in their strategic direction for the development of a resource centre and future use of accommodation on the hospital site. To identify the local requirements for each service, SHP facilitated a series of workshops for each service area. These were attended by clinical leads and service managers from the tPCT and local acute trusts, together with service users.
Process Plan for St Leonard's Community Hospital Redevelopment
The outcome of each of these work streams was:
An identified model of care;
Scope and scale of functional content;
Specification for service specific and shared facilities;
Estimates of Gross building areas.

The proposed development included the following:
Women's services: enabling routine antenatal services at St Leonard's and a Birthing centre for low risk deliveries.
Disability services: enabling the co-ordination of assessment of clients with complex health and social care needs and requirements for equipment.
Extended Primary care services: improving patient choice for care, more effective management of chronic diseases, reducing the need for hospital attendances and the development of enhanced primary care teams.
Sexual health services: providing satellite clinics in collaboration with local acute hospitals;
Diagnostic services: including local diagnostic imaging, blood taking for Pathology tests and other facilities;
Services for older people: providing accommodation for the integration of health and social care teams to deliver a challenging local care agenda.
The proposed Resource centre had a total space requirement of approx circa 10,000 square metres and included commercial opportunities for the provision of Cafe, Optician, Pharmacy and creche.
