East London LIFT Co
The East London LIFT Co was established in May 2003 following the selection of Global Solutions Ltd and Babcock and Brown as the consortium of private sector partners. It is a 20-year initiative to improve the primary and community services infrastructure across the East London and City area. SHP has worked closely with the LIFT CO and local PCTs across numerous initiatives. |
Greenhill Disabilities Centre and One-stop Primary Care Centre Development
SHP assisted Newham PCT in developing a business case for a new integrated disabilities and primary care development. The scheme was the result of the need to provide a holistic social care model for people with physical disabilities, who live in the borough.
The case was concerned principally with the development of integrated modern health and social care facilities, in the form of a disabilities and one stop primary care centre, with capacity to meet future local needs.

Vicarage Lane One Stop Primary Care Centre
In 2005 SHP developed a business case with Newham PCT outlining the planned new development and providing information on the services that will operate from it.
Improving the health of Newham's residents is the central objective in the development of the One Stop Primary Care Centre, it will primarily serve the population of Stratford, providing a centre for locality based clinics and access to primary care professionals. Integration with other local services, including Social Services and Newham University Hospital NHS Trust, will mean a joined up, seamless service for local residents. |

Barkantine Primary Care Centre
SHP assisted Tower Hamlets Primary Teaching Care Trust (tPCT) in developing a Business Case to support the capital development of innovative and fit-for-purpose primary care facilities at the Barkantine clinic site in the North Millwall area of East London.
The aim of the project was to develop integrated modern health and social care facilities with capacity to meet future local needs.
The new clinic will be a 5-story building on the existing Barkantine Clinic site and will be operated as a combined resource with the aim of providing a high quality and cohesive health and social care service.
The clinical facilities will be highly a ccessible by the local population, being placed on one of Tower Hamlets main thoroughfares. It will be well served by public transport. |
St Clements Integrated Health and Social Care Centre
SHP worked with Tower Hamlets tPCT and Social Services to establish the objectives of the scheme and to define the requirements for an integrated primary care centre, together with a Centre for Independent Living.
The proposal to develop a Centre for Independent Living will establish new ways of working together across health and social care. Service users will have access to a range of provision which will enhance their potential care pathway, maximise their re-ablement and enable them to learn skills which could facilitate them in accessing training, volunteering opportunities and employment. |

Child Development Centre
SHP has assisted City and Hackney PCT in developing a business case for the Hackney Child Development Centre. This innovative development is a joint venture between the City and Hackney Primary Care Trust, the Learning Trust, and the London Borough of Hackney. The aim is to provide an integrated facility which will be a major contributor to the health and wellbeing of children in Hackney, and help to bring together the public sector agencies as a "team around the child".
SHP led the development of the business case and in particular completed an analysis of the visions, service model and act ivity. An adjacency matrix and schedule of accommodation were created during a workshop led by SHP. |
Tower Hamlets tPCT Primary Care Centre Planning
SHP was involved in a series of workshops with user representatives in order to identify current and future clinical service provision, including capacity and flexibility/time share of clinical spaces and to agree schedules of accommodation for:
- Bethnal Green Primary Care Centre
- Gill Street Primary Care Centre
- Newby Place Primary Care Centre
These Primary Care Centres will provide a comprehensive range of health services. The tPCT vision is that the proposed care centres will become the hubs for the communities they service and that they will provide integrated health care. These proposals will also act as the catalyst to facilitate future service development, as appropriate.
